writing a report on siwes student industrial work experience

I need a format on how to write a report on student industrial work scheme
The Industrial Work Studies program (SIWES) is a skills training program designed to expose and prepare students from universities, polytechnics / technology colleges and education colleges for the industrial work situation they are likely to meet after graduation . The program also offers students the opportunity to become familiar with and expose themselves to the necessary experience in handling equipment and machinery that are not usually available in their institutions. It is a cooperative program of industrial internships that includes higher education institutions, industries, the Federal Government of Nigeria, the Industrial Training Fund (ITF) and the University Commission of Nigeria (NUC).

The Industrial Labor Students Unit (SIWES) of the Federal University Ndufu Igual Ikwo was established by the Vice Chancellor Oye Ibidapo Obe, FAS, FAEng, OFR in 2015 as an Industrial Coordination Unit in the Office of the Vice-Chancellor to Assist Students In industrial attachment.

The staff of the foundation was six (6) persons, composed of a Coordinator who is responsible to the Vice-Rector for the day-to-day business of the office and for the Administration, Coordination and Supervision of all its activities. The other staff includes an administrative assistant, a data entry lead helper, a desk officer, and an office assistant.

The objectives of the Plan are:
(I) Provide ways for students to acquire industrial skills and experience during their course of study.
(Ii) Prepare students for the industrial work situation they are likely to meet after graduation.
(Iii) To expose to the students the methods and techniques of work in the handling of equipment and machinery that are not available in the university;
(Iv) To provide students with the opportunity to apply their educational knowledge in real work situations, thus overcoming the gap between theory and practice.
(V) Facilitate the transition from school to the world of work by improving student contact for later placement.

Functions of the FUNI SIWES Coordination Unit

Under the mandate of the National Commission of Universities (NUC) and the Industrial Training Fund (ITF), the Unit is mandated to perform the following functions.
(I) Identify the placement opportunities for the student's link with the Employers.
(Ii) Supervision of students located in the industries located within our ITF zone.
Iii) Treatment of student logbooks, ITF forms, and industrial attachment reports on which Federal Government funding is based for student supervision and subsidies.
(Iv) Promotion of close links between the university and the industries involved in the SIWES program.
V) Provide advisory guidance to students participating in professional employment opportunities.
(Vi) Monitoring compliance of SIWES requirements by students in eligible disciplines as a condition of graduation.
(Vii) Facilitate the disbursement of the allocation of students to deserving students through electronic payment.
(Vi) Apply the prepared job specifications for all accredited courses and grant the appropriate credit units in accordance with the Federal Government's minimum academic guidelines.
Viii) Organize orientation courses in collaboration with the ITF for its students.
