1. What documents are required?
Complete the application form.
No. of AADHAR is not required, if the AADHAR number is not available, provide any of the passport / passport test / credential documents. The name of the applicant on the AADHAR card must match the name of the applicant.
Self declaration in the prescribed Performa.
The applicant's photograph will be captured through the webcam at the time the application is submitted or at the time of verification. The photograph of the applicant must match the photo of the applicant in the AADHAR.
Document of support for the current residential test such as Voter Card, Electricity Invoice, Water Bill, Telephone Account, etc.
Salary slip or other documents showing the applicant's family income.
Documents justifying the monthly expenditure.
IMPORTANT: All documents must be attested by the Gazetted Officer.
2. What is the cost of the application?
NO fee is required.
3. Where to apply?
You can approach the SDM in your area or you can also approach the
O / o the Deputy Commissioner from 10:00 AM to 1:00 PM on any work day.

4. What is the response time?
Certificates are issued in 14 days.
