I am writing to to request an abstract form for lost id. I lost my service Id on 28th May 2016. i just found out that i didn"t have it. I took a step further to repot the lost id card to the police station at Nakuru. I was given OB No. 42
Due to some inconveniences I was not able to take my abstact form there. I therefore request to download the same form.
