Most lung cancer patients are diagnosed when they have symptoms, have advanced stage disease, and curative treatment is no longer an option. An effective screening test for early detection has long been desired to reduce mortality from lung cancer. Sputum cytology, chest X-ray, and computed tomography (CT) have been studied as potential screening tests. The National Lung Screening Trial (NLST) showed a 20% reduction in mortality with low dose TC (LDCT) screening, and guidelines now endorse annual LDCT for those at high risk. Screening implementation is under way with the desire for benefits to be seen in clinical practice outside the format of a research study. Concerns include handling of false positives, costs, incidental findings, radiation exposure, and overdiagnosis. Studies continue to evaluate LDCT screening and the use of biomarkers in risk assessment and diagnosis in an attempt to further improve outcomes for patients with lung cancer.

Tests to diagnose lung cancer

If there is reason to think that you may have lung cancer, your doctor may order a series of tests to look for cancer cells and rule out other conditions. In order to diagnose lung cancer, your doctor may recommend:

• Imaging tests. An X-ray image of your lungs may reveal an abnormal mass or nodule. A CT scan may reveal small lesions in the lungs that may not be detected on an x-ray.

• Sputum cytology. If you have a cough and are producing sputum, looking at the sputum under the microscope may sometimes reveal the presence of lung cancer cells.

• Tissue samples (biopsy). A sample of abnormal cells can be removed in a procedure called a biopsy.

Your doctor may perform a biopsy in a number of ways, including bronchoscopy, in which your doctor examines abnormal areas of your lungs using a lighted tube that passes through your throat and into your lungs; Mediastinoscopy, where an incision is made at the base of the neck and surgical tools are inserted behind the sternum to sample tissue from the lymph nodes; And needle biopsy, in which your doctor uses X-ray or CT images to guide a needle through your chest wall and into the lung tissue to pick up suspicious cells.

A biopsy sample may also be taken from the lymph nodes or other areas where the cancer has spread, such as the liver.

Staging of lung cancer

Once your lung cancer has been diagnosed, your doctor will work to determine the extent (stage) of your cancer. The stage of cancer helps you and your doctor decide which treatment is most appropriate.

Staging tests may include imaging procedures that allow your doctor to look for evidence that the cancer has spread beyond your lungs. These tests include CT scans, magnetic resonance imaging (MRI), positron emission tomography (PET), and bone scans. Not all tests are appropriate for each person, so talk to your doctor about what procedures are right for you.
