There is no application form for student internship as such. You shall have to select the specific lab that you wish to work with and contact them directly.
The list of their various labs along with details can be found here--> drdo.govdrdo/English/index.jsp?pg=add.jsp
In general, you are required to apply through your college/institution, with an application letter stamped by your department HOD or principal, along with other documents that the lab may have asked for.
